As part of the re-opening process, the Department will be transitioning from having paper emergency cards on file at Community Centers to having paperless emergency cards through our CivicRec registration system. All current CivicRec accounts will be prompted to note any allergies or medications staff should be aware of. Also, please provide one emergency contact for each of your account members, including any adults. If your emergency contact does not have an email address, please input the email address that is linked to your account. Should you have any questions regarding your account or need any assistance, please contact the Recreation & Community Services Department at (909) 395-2020. Staff will be available to assist Monday – Thursday from 8:00 AM – 5:30 PM and Friday from 8:00 PM – 4:30 PM.
