In consideration for the right to participate in Grand Valley Recreation Center programs or activities, I hereby agree to the following:
I understand that any recreational activity, including independent activities at the Grand Valley Recreation Center or classes for which I am registering, involve certain risks to my or my child’s personal safety and property or the safety and property of others. I agree that it is solely my responsibility to ensure my or my child’s health is adequate and my or my child’s capabilities are sufficient to participate in the activities. I understand that I, or my child may be photographed and give permission for photographs to be used to publicize activities for the Grand Valley Recreation Center.
In the event of an emergency I give consent for me or my child to be taken to and treated at the nearest medical facility, understanding that every effort will be made to contact the emergency contact person set forth on registration forms and membership application forms. In such an event, I shall be solely responsible for all medical expenses associated with medical care. If I am signing this agreement on behalf of a minor I understand that the foregoing agreements apply equally to the child.
Parachute Battlement Mesa Park & Recreation District is a local government of the State of Colorado and must abide by all State and County Orders.
If staff members observe any violations, potential health issues, safety issues or behavioral , they will first provide the patron/customer with a warning.
After the second violation the patron will be requested to leave the facility and not be allowed to return for 48 hours.
After the third violation the patron will not be allowed to utilize the Recreation Center for 3 months.
Parents of youth will be contacted after the second violation and informed that they must address the issue before their child can return to the Rec. Center.
Patron also agrees to indemnify and hold harmless the District for all claims arising out of Patron or Parents refusal to comply with social distancing, health and safety orders or rules and regulations of the Grand Valley Recreation Center.
