COVID-19 Addendum: Participants should monitor for signs and symptoms of COVID-19. If you are experiencing any of the following symptoms, stay home and contact your health care provider. Any individual experiencing any of the following symptoms; • Fever (100.4F or higher), or feeling feverish, chills, • new cough, shortness of breath, • new sore throat, • new muscle aches, • new headache, • new loss of smell or taste SHALL NOT participate in NBPR programs or leagues. NBPR will be enforcing measures to prevent the spread of COVID-19. We request that if you do experience a confirmed COVID-19 case, you notify NBPR. NBPR will notify members if it receives information the employee was within 6 feet of a known infected person for an extended duration. NBPR may, at its discretion, withhold identifying information of the known infected person if that information is protected by Data Practices Statutes. Wash hands for at least 20 seconds with soap and water frequently throughout the day, but especially at the beginning or end of a program or when entering or exiting a facility, prior to any mealtimes and after using the toilet. Cover your mouth and nose with your sleeve or a tissue when coughing or sneezing and avoid touching your face, in particular your mouth, nose and eyes, with your hands. Individuals should dispose of tissues in the trash and wash or sanitize their hands immediately afterward. Participants are highly encouraged to wear a facemask when possible when entering participating. We understand that wearing a mask while participating in some programs is not optimal. Social distancing is being implemented in all programs and facilities through the following engineering and administrative controls: will be staggered at least 6 feet apart from each other at their workstations and are asked to maintain at least a 6 foot distance from their coworkers or participants at all times. Participants will be asked to stand at least 6 feet apart from staff and other program participants.
Zero Tolerance Policy: NBPR staff shall enforce a zero tolerance policy with regards to negative participant conduct while participating in programs, including but not limited to: refusal to comply with COVID-19 policy, smoking in restricted areas, and using, possessing or selling *alcohol/drug/or other illegal/age prohibitive substances, building equipment vandalism, theft, verbal and/or non-verbal harassment, profanity, rowdy or disruptive behavior, fighting, mischief, attire or gestures recognized as a gang representation will not be allowed. Individuals that are caught or suspected of the above actions will be required to leave the program, building and grounds. The NBPR staff shall be the sole judge of what constitutes the negative conduct or behavior covered by this policy. If necessary, NBPR staff will involve New Brighton police to enforce the zero tolerance policy and charges may be filed. Participants who exhibit negative conduct or behavior will be either temporarily or permanently expelled from programs or facility with no refund.
Assumption of Risk, Release, & Indemnity: I acknowledge receipt of the document titled “COVID-19 Addendum”. I also agree that as a condition of use of any City of New Brighton Parks and Recreation facility or participation in any program, sports league, class, recreation or fitness activity, you agree as a participant or parent/guardian of a participant in the program, to assume all risks and hazards of engaging in or as a spectator at any city activity or facility, including assuming all risks for personal injury, death and property damage. You further certify that you are properly prepared to engage in any activities you have selected and that you have recently consulted a physician to establish that it is safe and appropriate for you to engage in these activities. You further agree that there is there is risk associated in regard to the COVID-19 pandemic and possible exposure. Therefore, having read and understood this waiver and in consideration of your accepting my entry into a City of New Brighton recreation facility or program, I, for myself and anyone entitled to act on my behalf, waive and release the City of New Brighton, all staff, volunteers, sponsors, and representatives from all claims or liabilities of any kind arising out of my participation in (or as a spectator of) any program or use of any facility. YOU ARE AWARE AND AGREE THAT BY EXECUTING THIS WAIVER AND RELEASE, YOU ARE GIVING UP YOUR RIGHT TO BRING LEGAL ACTION OR ASSERT A CLAIM AGAINST NEW BRIGHTON COMMUNITY CENTER OR THE CITY OF NEW BRIGHTON FOR THEIR NEGLIGENCE OR FOR ANY DEFECTIVE PRODUCT ON ITS PREMISES. YOU HAVE READ AND VOLUNTARILY SIGNED THE WAIVER AND RELEASE AND FURTHER AGREE THAT NO ORAL REPRESENTATIONS, STATEMENTS, OR INDUCEMENT APART FROM THE FOREGOING WRITTEN AGREEMENT HAVE BEEN MADE.
